Appointment Information
To make an appointment with either FamilyCare location please call us at 828-669-4505. At that time, tell us what we can do for you. Same-day appointments are available, with appointments dependent on our caseload and the severity of your medical needs. Walk-ins are welcome at our Black Mountain office.
For First Time Patients
At check-in, you will be asked to complete a patient information sheet. The answers to this brief questionnaire will ensure we provide you the correct care today and in the future. Your personal information will be added to our database, which allows our staff to manage your care more efficiently and contact you when necessary.
What to Bring
Please bring your insurance card and a photo ID. Since most insurance companies require that co-pays be fulfilled prior to an office visit, please be prepared to do so. Payment for your examination and all related services is due upon the completion of your exam. If you have the need for special financial considerations, please relay this information when you check in.
